Can i do the medical check-up first, then when I'm within the 40 days I send the license and medical cert in for renewal? or must it all be within 40 days?

The 40 days requirement clearly states check-up must be done within 40 days before expiry. Check the circular in DCA's website for more details.

It's six weeks (42 Days) before the expiry of Medical Certificate. Check the AIC 09/2004 para 4.2

Yes you can do medical check-up first, however DCA will only accept the renewal of your license within 42 days from the date of expiry of your medical certificate.
