People in the 'sharp end' namely pilots, engineers and the like has been trained in Human Factors. We're are aware the consequences of FATIGUE, STRESS and other related HF issues that are related to human performance.
The onus is on that particular individual who should reconized the affect of fatigue and he/she should take the necessary steps and actions in order to reduce/minimize slips and mistakes.
Most of the pilots that I know are very professional and observed their rest period and working hours as per the required regulation to the 'T'.
I'd say this could be an isolated case. It may be rampant but we have no idea whats happening in Air India subject to further audit findings by the authorities.
I personally believe our safety standards are still high and well respected internationally. Just don't let our guard down and bend to commercial and operational pressures.
